Purchasing Products

Purchasing the Synergy V3 Products could not be easier, please follow the instructions below to get started.
Simply download and print the Autoship Activation Form which is located in the third column named "Product Shipment". Once you have the form printed you must write down the relevent information (see third column - Autoship Activation).

Once you have completed these steps your Synergy products will be shipped to you.

 

Product Shipment

1. Download The Form You can download the "Autoship Activation Form" by clicking the "Download form" button at the bottom of this column.

2. Print Form You must then print the"Autoship Activation Form"

3. Fill Out Form You must then fill out the"Autoship Activation Form". We recommend that you fill out the form under the guidance of your Synergy Distributor: Therese Dillon

4. Fax The Form Once you have completed the above steps you must fax the form to the fax no. at the top of the"Autoship Activation Form".
